Healthcare practitioners in India have to maintain a Daily Case Register with the following columns:

• Serial Number
• Date
• Name of Patient
• Nature of Professional Services Rendered
• Fees Received
• Date of Receipt

This is referred to as Form 25, and is an Income Tax requirement. Previously, this was referred to as Form 3C.
